A Total Occlusion of Left Anterior Descending Coronary Artery after Recurrent Atypical Kawasaki Disease

Abstract
An 11-year-old boy presented with prolonged fever of an unknown origin and multiple-joint pain for 15 days. When he had been six and eight years old he had complained of similar symptoms. At that time atypical Kawasaki disease was confirmed by two-dimensional echocardiogram, because dilatations of the left coronary artery were detected. A follow up had not occurred because he was asymptomatic. On this admission, the coronary angiogram showed a total occlusion of the left anterior descending coronary artery. Except the abnormal result of the cardiac SPECT showing a reversible perfusion defect all the results showed normal cardiac function. Because issues with parental permission made an immediate operation impossible, we felt we should observe him with a beta blocker, wafarin and aspirin for six months and see that no problems were found.
